May 23,2009 - 9:00 a.m. - 6:00 p.m./May 24, 2009 10:00 a.m.. - 5:00 p.m.
And May 25th 10:00 a.m. – 4:00 p.m.
GAME/FOOD APPLICATION – SHANLEY PARK
TERMS AND CONDITIONS:
Please read the following Terms and Conditions carefully.  Fill out form below and sign.  Submit original copy to:  HGMA, P. O. Box 2011, Granbury, TX 76048  (817) 573-5299
1. Merchandise and/or games, toys, rides, etc. by participants must be approved by the lessor, Historic Granbury Merchants Association (HGMA), HGMA reserves the right to require,
alteration, replacement and deletions of any material or activity, which is deemed inappropriate for this festival. All prizes given by vendors must have prior approval by HGMA.
(No live animals given as prizes)
2. Lessor reserves the right to change booth assignments at any time deemed necessary.
3. The undersigned hereby elects to and does release HGMA and its respective officers, agents and employees from any and all claims, demands,
rights or causes of action of whatsoever kind or nature which the undersigned has ever had or may now have or may hereafter have whether now known or unknown,
foreseen or unforeseen, arising from or by reason of or any way connected with any injuries, losses damages, property damage or loss, or the results thereof,
which heretofore has been or hereafter may be sustained by this undersigned as a result of or in connection with or arising out of the undersigned’s participation in the
Memorial Day Hometown Fair and Picnic
4. The undersigned acknowledges that in the case of rain, severe or hazardous weather or any other act of God there will be no reimbursement of booth fees or
any other monies submitted to HGMA or the City of Granbury.  The HGMA Committee will return no money of any kind or amount except in the case of booth space rejection.
5. Vendor will keep monies earned at this event and will not pay any booth charges for this event. (Any electrical fees above and beyond what may be provided by
HGMA will be paid by the Vendor).  However, all proceeds must go to a church, school or other non-profit organization unless otherwise approved by the HGMA Committee.
6. No electric or water is available in Shanley Park. If you bring a generator it must be WhisperQuiet.
7. Vendor agrees to pay any damages to Shanley Park that may occur during the event on the property occupied which could include ground rutting etc…
8. Vendor agrees to collect and pay any and all Texas sales tax, which is required to be collected for its goods and/or services.  Vendor further agrees to furnish
HGMA with a fully complete form W-9 when the executed Vendor Agreement is submitted to HGMA.
9. Vendor agrees to furnish HGMA with certificate of insurance or other evidence of liability insurance when the executed Vendor Agreement is submitted to HGMA.
